Make a referral
Our services are currently only available to adult residents of the London Borough of Ealing. We work with those who experience severe and enduring mental health issues.
We particularly welcome referrals where a mental health diagnosis has precluded access to other therapeutic or support services.
To make a professional’s referral into Cape services, please:
- Discuss the referral with your patient/service user
- Ask them about their current support/care needs
- Inform your patient/service user of the range of Cape services available
- Work together to identify how Cape services may help
- Contact us using the form below, using the ‘Your message’ box to briefly explain why the patient is seeking support from Cape.
Someone in the team will contact you within 7 days to arrange a professionals’ referral meeting (online or in-person).
Following the referral meeting, a full referral form will be sent via email on a secure link. This should be completed by the professional alongside the patient and submitted to Cape, inclusive of an up-to-date risk assessment.
If we can identify a more appropriate service for the case(s) presented, we will advise as such as soon as possible (usually within the referral meeting).

Once we have all the information required to accept the referral as a Cape client, we will aim to contact the client as soon as possible and no later than two weeks.
We will keep you informed on the completion of the referral (successfully contacting the client and scheduling the first meeting with them).
